FORM 1A-WHAT YOU ASK POTENTIAL NEW HIRES MAKING SURE THEY ARE NOT COMING IN WHILE THEY ARE STILL SICK FORM 1B-IS WHAT I WILL BE FOLLOWING UP FOR. ALL EMPLOYEES NEED TO READ AND SIGN FORM 1C-IS AN EXAMPLE OF THE CLEARANCE NOTE YOU WOULD GET FROM THE DOCTOR CLEARING YOU TO COME BACK TO WORK. POST THE VOMIT/DIARRHEA CLEAN UP POSTER IN THE BACK WHERE EMPLOYEES CAN READ IT. |
